In ons grote team van ongeveer 40 transferverpleegkundigen heeft iedereen naast de basiskennis van transferverpleegkunde een eigen specialisme. Jouw prioriteit: je regelt voor volwassen pati\u00ebnten de juiste zorg op de juiste plek, met de juiste zorgverleners buiten het ziekenhuis. Hiervoor zet je een groot netwerk in binnen \u00e9n buiten het ziekenhuis.<\/p>\n<p>Omdat we als academisch ziekenhuis zorgen voor pati\u00ebnten die meer dan algemene vervolgzorg nodig hebben \u00e9n uit het hele land komen, heb je contact met specialistische zorgteams uit heel Nederland.<\/p>\n<p>Als transferverpleegkundig team houden we ons bezig met verschillende vormen van vervolgzorg, zoals begeleiding, specialistische wijkverpleging, langdurige zorg, terminale zorg, geriatrische revalidatiezorg en eerstelijnsverblijf. Ook regel je zaken als medicatie, machtigingen, materialen en hulpmiddelen. Dit doe je binnen de kaders van wet- en regelgeving en vaak in overleg met de zorgverzekeraar.<\/p>\n<p>Iedere pati\u00ebnt heeft een eigen specifieke zorgvraag. Dat vraagt veel kennis en creativiteit, maar ook het vermogen om prioriteiten te stellen en goed samen te werken. Dat maakt het werk interessant en afwisselend.<\/p>\n<p>Om het werk als transferverpleegkundige goed eigen te maken, nemen we de tijd om je in te werken. Zodra je bij ons begint, sta je een korte periode als \u2018overgepland\u2019 ingepland. Je kijkt mee bij alle groepen: volwassenenzorg, kindzorg, psychiatrie en Specialistische Farmaceutische Thuiszorg (SFTZ). Daarna ga je zelfstandig aan de slag met twee vaste begeleiders aan wie je al je vragen kunt stellen.<\/p>\n<p>Wil je je verder ontwikkelen? Dat kan door op termijn de post-hbo-opleiding tot transferverpleegkundige te volgen. Denk aan het ontvangen, verzamelen en verzenden van pati\u00ebntspecifieke geneesmiddelen in de ziekenhuisapotheek. Ook beheer je de afdelingsvoorraad van de verpleegafdelingen en satellietapotheken en houd je hier toezicht op. Samen met je collega\u2019s streef je naar een optimale beschikbaarheid van geneesmiddelen voor pati\u00ebnten.<\/p>\n<p>De werktijden zijn variabel. Doordeweeks werk je tussen 07.30 en 17.30 uur. In het weekend en op feestdagen werk je tussen 08.00 en 16.30 uur.
